Personalized Supplement Plan

Your own personalized supplement plan created by a licensed pharmacist and expert in nutrition and supplementation! This plan is designed around your current health and medication lists to help you achieve your health and fitness goals desired. 

Upon purchase you will receive an email with a link to a survey. This survey will contain question asking what your fitness/health goals are, what medications/supplements you are taking and other pertinent information (lab results) that will remain strictly confidential! Once the survey is complete and all pertinent information is submitted, your personalized supplement plan will be delivered in a PDF format within 5-7 business days.  All of the supplements recommended will be available for purchase in our online store with a 10% discount for those who have purchased a supplement plan!
